Referrals

Requests for referrals may be done on line at info@eidushealth.com

Please include the first and last name of the provider, the provider’s id number for your specific health plan, and the reason you need the referral. You may also leave a message by phone with the same information on the referral message box.

Horizon, Cigna, and United referrals are written and can be picked up or mailed. Aetna and Oxford referral are done on the insurance web-sites. If you do this by email we will send you back the referral number for Aetna and Oxford to bring to the specialist. No referrals are faxed to specialists’ offices.
