If you wish, you may leave a message via email requesting an appointment and we will get back to you electronically to confirm. If requesting an appointment via email- please leave a brief description of the type of visit (eg- not feeling well, need follow up, need complete physical) and also leave times that you would be available. You must also leave your name and phone number.
